egistration with the practice
We welcome all new patients that live within our catchment area which includes the whole of Bursledon as well as the surrounding areas of Lowford, Old Netley, Butlocks Heath, Netley, Hamble, Swanwick, Sarisbury Green and parts of Hedge End and Sholing. If you are unsure whether you fall within our catchment area, please ring the surgery and we will be able to advise.
In order to register with us, please print out our New Patient Registration Document below and bring it in to the surgery. When you bring these in you will also need to complete a purple GMS form which will help us trace your medical records. Please bring with you two forms of ID (one photographic and one proof of address - please note you cannot use a photo driving license for both forms of ID). Please fully complete each section of the form as this is information required to register you. Any missing information can cause a delay to you being added to our system.

Our Catchment Area
Below is interactive map of Bursledon Surgery's practice boundaries. You can put your address in the search area to see if it is within the practice area.
The area in blue in the middle of the map is the main practice area.
The area in yellow to the left and upper area of the main practice area is the practice's outer limits. Patients who are registered in the main practice area but have moved to the outer limits area can continue to be registered as a patient at Bursledon Surgery. However, any new patients living in the outer limits area who are not currently registered with us will not be able to register at the practice.
